Since its introduction in 1980, the percutaneous endoscopic
gastrostomy (PEG) procedure has gained
widespread popularity as a means of providing nutritional
support to patients who cannot maintain adequate
oral intake. 1 PEG tubes were developed as an
alternative to surgical gastrostomy and can be inserted
safely, rapidly and with less cost than the surgical
techniquefi 3 However, major complications, including
gastric perforation, hemorrhage, and severe aspiration
occur in about 3 % of procedures; minor complications
such as wound infection, ileus, stomal leak,
and tube migration occur in about 13 % .4-6 Erosion of
the internal bumper into the gastric wall, the so called
"buried bumper syndrome," has been described more
recently. 7 We report our experience with nine patients
who developed the buried bumper syndrome as a
complication of the PEG procedure, and describe endoscopic
approaches used to remove the feeding tube
and provide suggestions for prevention
